Texas PASRR form 1013 replacement

What’s next after Form 1013 for Texas providers?

Back in May, Texas Health and Human Services (HHSC) announced the discontinuation of Form 1013, used when a nursing facility needs to change a negative PL1 that has already been submitted.

In the Aug. 29 Texas PASRR Technical Assistance Call (PTAC), HHSC discussed the replacement form for the 1013 that will be used to address Mental Illness and Dementia. If you missed the live webinar, be sure to watch the recording to get this important info for Texas nursing facilities.

  1. Hello, I am unable to find the Form 1012. Is it on the HHSC website, TMHP or somewhere else?
    Thanks
